Basic Model of Information

1) Responsible for Data Processing.

IQVIA Inc. and/or its group affiliates (IQVIA) is the entity responsible for processing any personal data that is collected through the HCP Profile form found at https://*.iqviaconsent.compltech.iqvia.com.

2) How to contact us.

If you face any issues or concerns providing your information on the HCP Profile form, please contact the IQVIA data protection officer at PrivacyOfficer@IQVIA.com.You can also find a list of toll-free phone numbers in the IQVIA toll-free phone list.

3) Purpose of treatment and legitimization of treatment.

The completion of the form is voluntary. However, if you decide to withhold your information, you will not be able to register on the IQVIA Consent platform and benefit from its services.

If you tick the check box under the section Basic Model of Information, you authorize IQVIA to process your data for the following purposes:

- Manage your registration on the IQVIA Consent platform. This will enable the platform to request your consent on digital documents and use previously saved signatures, codes or passwords.

- Provide the services of IQVIA Consent to pharmaceutical companies that contract with IQVIA for the use of the platform on Consent Management or signing documents, with the purpose of fulfilling the obligations of transparency required by the regulations of the medical, health and pharmaceutical sector. Other types of documents and / or authorizations that require consent or a signature may also be included.

4) Recipients of the data.

IQVIA will not communicate your data to third parties, except our third-parties service providers, to comply with the legal obligations or with your permission.

5) International data transfers.

Data is hosted on servers located in the IQVIA offices in Boulogne, France.

6) Data retention.

IQVIA will retain your data if the purpose of treatment remains valid, subject to your right to request the deletion of your information.

7) Rights.

You can request and exercise your rights to access, rectify and/or delete your information. You also have the right not to be subject to a decision based solely on automated processing, including the preparation of profiles. You can exercise any of these rights by contacting us at Gilead_Privacy_ITA@gilead.com.You must attach a photocopy of your identity card and/or any other legal document that can prove your identity.

If you are an EU, Swiss or UK citizen and feel that IQVIA is not abiding by the terms of this Policy, please contact IQVIA via the contact information provided under Section 2 How to contact us. If any request remains unresolved, you may contact the national data protection authority for your country.
